Anytime the Undo History window is open it breaks the group selection by keyboard functionality. IOW, I can enable keyboard focus to the group bin and enable/disable groups by pressing the group ID letter on the keyboard, BUT if the Undo History window is open anywhere on the screen, this functionality is broken!
Confirmed on other systems.
When inserting the TC Electronic LM2 (latest version), Pro Tools crashes. It just vanishes without any error message.
Seems that in Pro Tools Ultimate 2018.4, tabbing to the previous transient (Opt+Tab) jumps backwards at a random rate.
My Main Output is a 5.1 Output. When setting my Audition Path to that, it seems to 'downmix' to a mono output on Channel 3 (Centre) when previewing in any AudioSuite Plug Ins.
Workspace Auditioning is fine though & plays out of L&R. Surely that's the same Audition Path?

Serato Pitch'n'Time Pro crashes pro tools.
Sometimes it works as it should, other times it will crash Pro Tools on a certain region.
It is very frustrating.
I'm mainly looking for a support update for PT12.
Sometimes when I load a project containing an Avid Space (AAX 12.0.1 or 12.1.0) plugin I notice that the Wet control has mysteriously shifted to 0dB and the dry control to -infinity dB, no matter what the previous setting was.
Seemingly random, have not been able to replicate.
Highlight multiple files for renaming. Shift-Command-R to rename. Use A, B, C naming conventions. Notice that 1st file is named B, second is C, third is A.
See video. Please fix this. No longer possible to trust Avid's rename function for more than one file at a time.
If Pro Tools is running when macOS goes to 'Sleep' (e.g. when closing the lid of a MacBook Pro) it will crash completely upon resuming the OS, presenting the standard 'Send to Avid' option.
Known to occur in Pro Tools 12.7.1, running on macOS 10.12. Unknown if impacting other versions.
Sometimes a terminal suddenly pops up.
As it looks, it caused by some bug in the interaction between OS and Avid apps.
Maybe it's the Application Manager that causes all this trouble?
